Consultant Forms
Consultants approved by the BOH are allowed to witness perc tests, bottom hole (SAS Leaching) inspections and/or final inspections should the health agent be unavailable to do so. For final inspections, the consultant will complete our "Consultant checklist for Final Insp" and submit it to the BOH.

Our consultant fee for bottom hole inspections, final inspections and re-inspections is a flat rate of $60.00. The BOH requires the hired consultant to complete our consultant billing form posted above.
The consultant will at the time of service give to the homeowner/resident/hiring party a copy of the BOH “Consultant Billing Form.”
The homeowner/resident/hiring party will in turn pay the Town of Halifax BOH the consultant’s fee for the services listed on the “Consultant Billing Form.”
The consultant will deliver or mail to the BOH their part of the “Consultant Billing Form” and the BOH will submit the bill for payment once payment is received from the homeowner/resident/hiring party.
***At NO TIME should a fee for services rendered exchange hands between a homeowner/resident/hiring party and a consultant approved by the BOH. ***
